The form states: "Fees: $30 for Application & $30 for Permit ($60 Total Due)." Submission instructions read: "To Submit, download, Print, Fill Out and Email to [email protected]." The form asks each listed General Contractor, Plumber, Engineer, and Electrician to state whether they are "Licensed" and "Insured," and separately states: "Contractors, please attach or email proof of insurance ([email protected]) Permit will not be issued until it is received." All State Permits and Local Building permits "must be posted on the property before starting the project." No page names an adopted building code edition.
